Welcome to the website for the University of California‘s Medical Plan Satisfaction Survey. UC values the opinions of our medical plan participants.

To enter the survey, enter the personal code from the letter you received. The code is in bold in the middle of the page. If you received an email instead, the code is under the “Begin Survey” button.
